TO BEGIN:
Gather the 7 items below. Once you have collected all of these items, you will need to take them to a post office or county clerk to get the application authorized and sealed by a "passport acceptance agent." To find an acceptance agent near you, you can search by zip code by visiting http://iafdb.travel.state.gov/. Both parents and child must appear in person together, or 1 parent may appear with the child and a signed and notarized consent form (DS-3053) from the parent who is not appearing. If one parent has sole custody, a consent form is not needed. An original court order proving sole custody must be submitted unless the birth certificate names only 1 parent’s name on it. Both parents must have a state ID or driver’s license to present to the agent. The agent will administer an oath, authorize your child’s DS-11, direct you to sign the DS-11, and put the application materials in an envelope and put their official seal on the envelope. If the agent does not want to put ALL of the below items into the envelope (such as the copy of ID), that is OK, but it is important that you send them to us anyway. They will then return the envelope to you. If you are getting more than 1 application sealed, it is important that each application is in its own sealed envelope. Multiple applications in 1 sealed envelope may be denied.

1. DS-11 Passport Application
|
This application must be properly filled out online according to the U.S. Dept. of State instructions.It is essential to click "Create Form." Doing so will create two bar codes, one at the top left and one on the right side.To fill out this form go to the following link: DS-11 New Passport Application
Must be signed in front of a passport acceptance agent
Must be signed in black or blue ink
Must be free of crossed out items and white-out
Must be filled out completely with your social security number

2. Proof of Citizenship
|
You must provide an original state issued or official copy of your US Birth Certificate, your Certificate of Naturalization, or an expired passport that is in good condition. This original must be included in the sealed envelope. This document will be returned to you with your passport.
Photocopies are not accepted
Birth Records are not accepted (these often are small with hand and footprints)
For newborns (younger than 3 weeks), you may use the birth certificate issued at the hospital. MOST of the time this is accepted, but the passport will be issued for 1 year validity rather than 5
US citizens born abroad will need to include a birth abroad certificate

3. Two Identical Passport Photos(Kinko’s, Walgreens, CVS have services)
|
Must have been taken within the last 6 months and represent your current appearance
Must be 2"x2" with a white background, and in color
Homemade Digital Photographs will not be accepted
Ears need to be visible and headbands are not permitted
No dark glasses
Must be facing forward
For infants, parents hands may not be visible in the photo

4. Proof of Parents Identification - Copy of current valid driver's license, government ID, state ID, or Military ID
|
Photocopies are accepted- must be able to see photo and ID number
If either parent is not a US citizen, please submit a copy of each parent's passport and a copy of the visa or green card associated with the foreign passport
If either parent's name has changed and is different than the name on the birth certificate, original documentation proving name change must be sealed inside the envelope (divorce decree or marriage certificate that shows name change)

5. Payment to Government Make one personal check of money order payable to "U.S. Department of State" for $140
|
Temporary or starter checks are not accepted
Write the applicant’s name and date of birth in the memo section of the check
Please note that if the agent prompts you to write a check for an amount other than $140, the check will not be accepted, and your application will be delayed until a replacement check is provided

6. (2) Original Swift Passport Services Letter of Authorization
|
This letter will authorize Swift Passport Services to submit and obtain your childs new passport and act as an agent on your behalf. 2 copies with parents original signatures are needed – One goes inside the sealed envelope, and one should be attached to the outside Click Here To Download Swift Passport Services Letter of Authorization

7. Proof of Departure
|
One of the following must be submitted on the outside of the sealed envelope
A photo-copy of your airline ticket, a copy of your itinerary from your travel agent or airline
If you do not have a ticket or itinerary, a letter explaining business travel can be submitted. Follow the sample "company letter of expedite" format. If you are using a company letter of expedite, please indicate this when placing your order
A letter explaining that you will be driving to Canada and/or Mexico. Include intended departure date, intended return date, and destination
