Apply for New or First Time U.S. Passport. You qualify if the following criteria are true:

• You never had a passport and are applying for a U.S. passport for the first time; or
• Your expired US passport is not in your possession; or  
• Your previous US passport has expired and was issued more than 15 years ago; or  
• Your previous US passport was issued when you were under 16 years old

Apply for a Passport Renewal. You qualify if the following criteria are true:

• you can submit your most recent, undamaged passport; and
• were at least 16 years old when your most recent passport was issued; and
• you were issued your most recent passport in the last 15 years; and
• You still have the same name as in your most recent passport (or you can legally document your name change). 

Apply for a Child PassportYou qualify if the following criteria are true:

• You/your child is age 15 or younger
• If child has had passport before, use this application 

Apply for a Lost or Stolen PassportYou qualify if the following criteria are true:

• You are unable to locate your current, valid passport; OR  
• Your passport has been stolen, and it is still valid; OR
• Your last passport was issued within the last 15 years and it is not in your possession

Apply for a Additional Visa PagesYou qualify if the following criteria are true:

• Your current passport is not damaged
• Your current passport has at least 12 months validity remaining

Apply for a Passport Name ChangeYou qualify if the following criteria are true:

• You have legally changed your name since the issuance of your last passport
• Your identifying information on your passport is printed incorrectly 

Apply for a Second Limited or Second Valid U.S. PassportYou qualify if the following criteria are true:

• You can submit your most recent, valid, undamaged passport (which will be returned); AND
• You are traveling to a country where your current, valid passport will not be accepted due to Visas or entry stamps; OR
• You need to travel while waiting a long period for a Foreign Visa to be issued
